Hu antibodies


Suspicion of paraneoplastic syndrome.


Immunoblot and indirect immunofluorescence (IIF). The antigen for the immunoblot is produced and purified by biochemical methods. The indirect immunofluorescence (IIF) test is a combination of fixed tissues from the nerve, cerebellar, intestinal tissue and pancreas originating from monkeys. In cerebrospinal fluid only the immunoblot test is done.


The test result is expressed as negative or positive. IIF followed up by answers in titer and immunoblot is followed up by answers with intensity. For CSF the answer is either positive or negative.


Results for paraneoplastic antibodies should be verified by both analyzes to provide a comprehensive response according to European guidelines.
According to the kit supplier the assay sensitivity is 100% and specificity is 100%.
Anti-Hu is sometimes referred to as HuD and is also called known as ANNA-1 (anti-neuron specific cell nuclear antibodies). The antigen is present in all neurons in the central and peripheral nervous system and in several tumor types primarily in the lung. Anti-Hu is present in paraneoplastic subacute sensory neuronopathy and / or paraneoplastic encephalomyelitis. Within this patient group  83% also have small cell lung cancer and sometimes also other types of cancer (breast cancer, Hodgkin's lymphoma, malignant melanoma, ovarian cancer, testicular cancer, thymoma, etc.). Small cell lung cancer is occasionally associated with Lambert-Eaton myasthenic syndrome which also has auto-antibodies towards voltage gated calcium channels (VGCC). The paraneoplastic syndrome patients often develop neurological symptoms before clinical manifestations caused of the tumor. In the treatment of paraneoplastic syndrome it is important to find the underlying tumor and treat it.
Generally, treatment with immunotherapy is more effective against auto-antibodies directed to extracellular antigen than with auto-antibodies against intracellular antigens.
